Insurance Guidelines

  • We will verify that your insurance is effective and verify your benefits.
  • We will communicate the information we receive from your insurance company to you before beginning treatment.
  • We will submit your services directly to your insurance company and assist you in monitoring your claims.
  • Deductibles, coinsurance and co-payments are due at the time of services.
  • We will file third party claims (auto and personal injury). If claims are denied, the patient will be responsible for their balance.
  • Patients should be aware of their insurance policy terms.
  • Prior authorization/ PCP referral is needed to begin evaluation and treatment.

This list is not exhaustive. We will accept assignment from insurance companies for PPO and non-PPO payors. We will call insurance companies and pre-certify services before patients are seen. We are constantly expanding our insurance contracts. If you do not see your insurance company listed, please call us regardless.
